help, or see other options below.
Do not hit "enter" or the form will submit
before you are ready.
You may use "tab" to move between fields.
You can (should) print the screen
before you attempt to submit it, in case it doesn't go through so you don't loose the information.
Make sure you have not blocked pop ups in your security, or that you
have set your browser to allow pop ups from my website. Once submitted, you should get a pop up confirmation screen that says it went through.
Some people are not getting the pop up confirmation,
but the form is coming through to me.
I send an email confirming the receipt of
forms and appointments when I get them. This is done by hand (not auto response) so please allow time for that to happen
since I do not check email 24 hours/day.
Please be sure to hit the "Submit"
button at the very end of each of the two sections, or I won't get the form!
I have not yet confirmed, but suspect this may have something to do
with Vista or Windows 7 settings, but it may just be the pop up blocker of your security or browser.
You may have a problem with a firewall on your computer, so you might try adjusting
your firewall settings. If you can't submit a form, pleases disable your firewall
software and try again.
If you dodn't know if it went through, but did not get an error message, it
is best if you allow a little time for me to send you email confirmation but you can call my office to see if I
got it.
If your form did not come through, I will ask you to fax it to me, mail or email a
copy, or just bring it with you to your meeting. As recommended earlier, you should have printed the form.
Please do NOT fax it to me until we are sure it did NOT come through via email since I am getting almost all forms,
unless an error message is displayed. My fax is 704/377-0775.

*IMPORTANT NOTE* Some people are experiencing difficulties with submitting this form due to firewalls.
I am working with the site host to fix the problem but I recommend that before you submit, you print this page. If it goes
through, you will get a confirmation screen that says it went through. If you get a message that says "java.lang.NullPointerException"
, then it did not go through. For information about resolving the problem, please follow the links on the
