This form is for people who have already spoken to Ms. Robicsek and have a scheduled appointment with the Law Office of Susanne M. Robicsek. 
 
This form has been made available by request from past clients who wanted to reduce the amount of time they spent at the first consultation.   This should not take very long - ten to twenty minutes.  If it takes longer, you may be making it more difficult than it should be.  This should be based upon your best estimates;  guess if necessary.  The form will be used to get an idea of what your ongoing living expenses are, and will be reviewed with you in your consultation, and helps me tailor the explanation of bankruptcy to your particular situation.  The less time I have to ask questions about these items, the more time I have in your consultation to answer your questions and explain how bankruptcy might affect you.
 
If you prefer (or have trouble with this form), a paper copy can be given to you to fill out when you come in for your appointment but you need to let me know prior to coming in so that your time with the attorney is correctly scheduled. 
 
You can type numbers for the amounts, and you can also type  words if you need to explain or describe something.  If it doesn't apply, type "0" so I know that you didn't miss the question. 
 
CREDITORS AND DEBTS:
 
Please provide a simple list of creditor names and balances (no pennies needed) for all your debts.  No mailing addresses, account numbers, or credit reports please, for this consultation form.  If you proceed with filing for bankruptcy with me, I will ask for that information to prepare your case.
 
INCOME:
 
Please note that I want to see your gross income (before taxes) and your take home.  If you have any retirement, disability, part time, etc, provide those figures as well.
 
EXPENSES:
 
This form asks for average monthly expenses.  I assume any amount you put down is a MONTLY amount however not everything is paid monthly.  For instance you may not pay for medical costs, buy clothes, repair your house or your car every month.  Something like tires only might be bought every other year, but you need to factor those in too.  You may calculate of what you spend per year, and divide by 12 to give me a monthly figure as requested, or if is easier write in what you spend yearly just type "yearly" or "6 months", etc. next to the amount so I know you are not using a monthly figure.   Example:  car insurance  $650/6months.  Example:  clothing:  $300/year.  Example:  car repairs:  $2500/year.  Example:  Homeowners:  $75/qtr.
 
TIPS:  Do the best you can.  This is not a test.  The purpose it to see what you really need to spend to live, and it is a starting place for our discussion.   I review this form in the consultation and it will be updated or changed as needed, as we review it. 
 
1)  Be honest, and as accurate as you can. 
2)  Don't shortchange yourself.  List what your expenses really are, or should be.  If they appear unreasonable, I will discuss that with you.  For example, many people write $0.00 for clothing because they have not bought any in so long.  Everyone wears clothing, so it has to be bought by everyone at some point so that needs to be budgeted in.
3)  Do not list payments on credit cards, or on unsecured loans that have no collateral in the expense section.
